We provide support to Elderly, End of Life, Learning Disabilities and Autism, Dementia Care, Respite Care and Adults over the age of 18 years. 

We provide a wide range of professional services to meet your needs. We understand the vulnerability of people that require support and this ensures we provide the highest level of quality care, amongst our other values. 
PLEASE BE AWARE
Prepare4care do not discriminate and provide both male and female Care Specialists. We are happy to meet needs where possible but if you are not happy to have a male Care Specialist this must be stated on enquiry.

    Assessment

Before you receive any Care Support from Prepare4care, we come out to your home at your convenience, in order for us to find out what you want and how you want  us to support you.  It is a great way for you to get to know us and for us to get to know you. 

The assessment is the first stage in your Prepare4care journey, this allows us to tailor a package that suits all your needs and is flexible around you.  After your assessment with us, your support plan is developed to be completely unique to your needs, lifestyle and your routines based on the information we have gathered.

Our Home Care package is tailored to suit all your needs and is flexible around you. This can range from personal care support, to social aspects, to meal preparations, shopping/domestic duties, support with medication and more. 

Home Care can be frightening for some people, especially when you have lived such an independent life. Our Care Specialists are not there to take that independence away but simply support the life you already lead. 

Changing a person’s environment and routines can be very distressing, even more so when that person has been diagnosed with Dementia. Dementia can make the world confusing, stressful and frightful, that’s why we believe that keeping you in your own home, now more than ever, is the better choice compared to Residential Care. There’s nothing better in our eyes than a person who can stay in the place they know and love. That way you will feel secure, surrounded by your precious memories and worldly comforts.

Non Nursing packages

You may be a main Care provider for a loved one or you may have just come out of hospital. Whatever the reason we can provide short or long term Respite Care.

Our Respite Care packages will give that much required rest break to full time care providers, while they have the knowledge that their loved ones are in safe hands. 

Our Respite Care package could provide you with temporary quality support, while you are recovering and healing.

Our Care Specialists can provide care and support for individuals that are near the end of their life. 

End of Life is a difficult time for the person and their loved ones. All of our Care Specialists are fully trained in Palliative Care and will support the requirements in place for a dignified, and comfortable death. 
This will allow your loved one to stay in the familiar and comfortable surroundings of their own home.

Live in Care and Support

24/7 support package

Our Live in Care package is perfect for you if you need and want 24/7 Care support. 

This package will offer that one to one, 24/7 Care support, full time security and safety to you and your family, that a timed Care service may not provide. 

It can be a great alternative to residential care and will keep you surrounded by the comforts/familiarity of your own home.
